New Patients
Please bring any insurance information (cards, claim numbers, etc.) that you may wish our office to bill and/or check for coverage. If we are not in your network, most of the time we can meet or beat your out-of-pocket costs.
Please allow one hour for your first appointment. A thorough exam will enable us to consider the appropriate treatment protocol for your specific condition. If required, follow-up visits will require less time
New Patient forms will be required to be filled out which contain questions that pertain to the nature of your visit to our office. Arriving a few minutes early to fill out necessary paperwork would be appreciated
The visit will begin with the doctor noting a comprehensive history of the patient's condition. This will include the onset of the condition, past medical treatments related to this condition, as well as the patient's family history. This survey will be followed by a comprehensive examination. The exam will include postural screening, Range of Motion evaluation, neurological exam, appropriate orthopedic testing with palpation of the afflicted areas. Dr. DeBoer will assess if x-rays are required for further diagnosis and the x-rays will be taken in our office. If the doctor deems it appropriate and if treatment is agreeable to the patient, treatment will then be rendered.
